Insurance & Getting Started

Know your coverage before your first visit.

We work with most PPOs, Medicare, and Workers' Comp. Our billing partner, Prava Therapy, checks your benefits for you and emails you a summary of what's covered and what you can expect to pay.

Who Handles What

Prava handles the paperwork. Dr. David handles your care.

Prava Therapy

Your contact for insurance verification, scheduling, and billing. They'll reach out to you directly for these admin tasks. Mention Dr. David Waltman so they connect you with him.

Dr. David, DPT

Check Your Benefits

Send your information the way that's easiest.

Once Prava has your details, they'll run your benefits and email you before your first visit.

Your Benefits Summary

What you'll find out before you start.

  • Whether your plan is in or out of network
  • Whether your plan covers telehealth visits
  • Whether your plan needs a referral to get started
  • Whether prior authorization is required
  • Your estimated out-of-pocket cost per visit
By Plan Type

What to know about your insurance.

PPO plans

We work with most PPOs. Your benefits check confirms whether you're in or out of network and what your copay, coinsurance, or deductible looks like.

Medicare

Covers physical therapy for pain, injury, and surgery recovery. Cash-pay programs aren't available during your rehab. Once rehab is complete, you'll fill out a short form, and then you can continue with cash-pay wellness and performance training.

Workers' Comp

Include your claim number and adjuster's contact information when you send your details. Prava will confirm what your claim requires, including any authorization, before your first visit.

Referrals

No referral needed to get started in California.

California lets you see a physical therapist directly. Some insurance plans still require a referral before they'll cover visits, and your benefits check will tell you if yours does. If you already have a referral or doctor's order, send it to Prava with your insurance cards.

Prefer to pay cash?

Performance and longevity programs are cash-pay, starting at $680/month for weekly 45-minute sessions. Medicare patients can join once rehab is complete.
